Ayushman Bharat PM-JAY Mukhmantri Sehat Bima Yojana (AB PM-JAY MMSBY), was launched on 20 August 2019 this is a flagship State Health Insurance Scheme for the beneficiaires of State of Punjab. AB PM-JAY MMSBY will provide financial protection to 65% population of state of Punjab. It is entitlement based cashless Health Insurance cover. Under this scheme cashless and paperless treatment is available at Government and empaneled Private hospitals.
Mukhyamantri Sehat Bima Yojana Key Highlights
- Cashless and Paperless Treatment: Under this scheme, beneficiaries can avail cashless and paperless treatment at both Government and empaneled private hospitals.
- Coverage of ₹5,00,000 per family per year: This cover applies to secondary and tertiary care hospitalization for treatment of listed diseases and medical conditions, ensuring comprehensive healthcare protection.
Features of Ayushman Bharat PM-JAY Mukhyamantri Sehat Bima Yojana
1. A cashless health insurance cover of Rs 5 Lakh per family per year.
2. Cashless secondary care & tertiary care treatment (1579 packages) in public & empaneled private hospitals in all states of india.
3. 187 packages reserved for Government Hospitals.
4. Entitlement based Scheme.
5. Pre-existing diseases are covered and treatment package includes 3 day’s pre-hospitalization and 15 day’s post hospitalization expenditure.
6. Beneficiaries can avail health services at empanelled private and government hospitals in all states of india where this scheme is being implemented
Mukhyamantri Sehat Bima Yojana Eligibility Criteria
The scheme is available for the residents of Punjab, and applicants must fall under one of the following categories:
- SECC 2011 data families
- Smart Ration Card holder families
- J-Form holder farmers
- Small traders registered with Excise & Taxation Department
- Small & Marginal Farmers
- Accredited & Yellow Card Journalists
- Construction workers registered with the Punjab Building & Other Construction Workers Welfare Board
Mukhyamantri Sehat Bima Yojana Online Application Process
The application process is simple and can be done online:
- Download the Ayushman App on your mobile or visit the official NHA Beneficiary Portal for self-registration.
- After successful registration, you can visit any empaneled private or public hospital (hospital list available on the SHA website) to get your e-card generation.

- Aadhaar Card
- Ration Card (or Family Declaration if no Ration Card)
- Income Certificate
- Construction Worker Card (for construction workers)
